Form for faculty members seeking license approval to use radioactive materials. All use of radioactive material must be done under the supervision of a Licensee who has been approved/licensed by the Radiation Safety Committee (RSC). Each Licensee is responsible for the health and safety of persons using RAM under his/her license, assuring that workers are properly trained, and assuring that work is done in accordance with relevant policies and procedures.
